                                                     Before calling:

 

  • Please bring your insurance card.
  • Please obtain a referral if your insurance plan requires one.
  • Accepted insurances:

                    - All insurance is accepted but we may not be in network with all insurance companies, so please contact your insurance company to verify whether our office is in network.

                    - We are members of most HMO/PPO carriers.

                    - The patient is responsible for knowing the requirements of her insurance.

We accept all types of Medicaid (Note that some Medicaid plans require prior authorization before your appointment. Please contact our office for more information.)

 

 

 

Preparing for an appointment:

   

  •  New patients - Please print and complete all of the forms in the new patient packet located in the medical forms tab on the left side of your screen.  Bring them with you on the day of your appointment.  You must also submit a copy of your insurance card, and a photo ID (i.e.: Drivers License, Military ID, or Student ID). We ask that new patients arrive 30 minutes prior to their first appointment. 

 

  •  Returning paitents - If you are coming in for your annual exam please print out the appropriate forms located in the medical forms tab on the left of your screen, and bring them with you to your scheduled appointment.

 

 

Payment is due at the beginning of your appointment. Accepted methods of payment are:

                          -Cash

                          -Credit Card (Visa, Mastercard)

                          -Check

 

  • Please note that if you do not have insurance and cannot pay the full amount, we will collect a minimum of $100.00 at the time of service.

 

 

Canceling Appointments

 

In the event you are unable to keep your scheduled appointment, please call our office in advanced to cancel. If you fail to notify the office of a cancellation in a timely manner, you will be charged a "no-show" fee of $30.00.  If you do not show for your appointment for three consecutive visits you can no longer be seen by a Bloomington Ob/Gyn provider.

 

 

                                   Appointments for Minors

 

Patients under 18 must have the consent of a parent or guardian in order to receive treatment from one of our providers. This person must accompany the minor patient to the appointment.

  

Unscheduled Visits and Emergencies

 

Medical emergencies are given immediate attention.  Patients that visit or call the office for routine care without a prior appointment will be assisted as time permits during the course of the day. Walk in patients will need to schedule an appointment if it is not an emergency. Please note that our office gives scheduled patients our first priority.
