| Fees Fees are based on reasonable charges. An estimated fee for your procedure will be given on request. All co-pays are expected at the time of service. If you are unable to pay in full at the time of service, prior arrangements MUST be made with our Financial Counselor BEFORE services can be rendered. If your service is covered by your insurance company, all co-pays and deductibles must be met. If you do not have insurance, we expect payment in full. If payment in full cannot be done at the time of the appointment, a minimum of $100.00 dollars will be collected. You can also call our office to make a payment. Past Due accounts If you have a financial problem, we understand. Contact the Financial Counselor at extension 204 to make arrangements for payments that accommodate you and the office. If you do not make these arrangements, or fail to make your payments, your account will be sent to collections. Insurance plans We currently accept almost all insurance plans. If you have any questions, please contact our office at extention 204 to verify our participation in your plan.
